Responsibilities
- Provide remote patient monitoring and care coordination.

- Coordinate with healthcare providers, specialists, and families to ensure quality care.
- Document patient encounters and maintain accurate electronic health records.
- Participate in regular team meetings and training sessions.
- Adapt to new technologies and platforms as needed.
- Provide exceptional patient care and support.
Qualifications
- Current RN license in the US.
- Minimum of 2 years of clinical experience.
- Proficient in English, both written and verbal.
- Strong computer and technology skills.
- Ability to work independently and manage time effectively.
- Empathetic and excellent communication skills.
- Ability to work flexible hours, including evenings, weekends, and holidays as needed.
- Experience in telehealth or remote care is a plus.
