Responsibilities
- Assess patient health conditions remotely via telehealth platform.
- Develop and implement care plans in collaboration with healthcare team.
- Educate patients and families on health conditions and treatment plans.
- Document patient encounters and progress notes accurately and thoroughly.
- Coordinate care with other healthcare providers and specialists.
- Provide exceptional patient care and support.
- Adhere to strict confidentiality and HIPAA guidelines.
- Maintain a safe and organized home work environment.
Qualifications
- Current RN license in the US.
- Minimum 2 years of clinical experience.
- Strong computer skills and proficiency with electronic health records (EHR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Access to a reliable internet connection and a private, quiet workspace at home.
- Experience with telehealth or remote patient monitoring is a plus.
- BSN or MSN degree preferred.
